واکاوی تجربۀ زیستۀ معنوی مردان مبتلا به سرطان کولون

چکیده
زمینه: سرطان کولون، به‌عنوان یک بیماری پیچیده، ابعاد جسمی، روانشناختی، اجتماعی و معنوی بیماران را به‌شدت تحت تأثیر قرار می‌دهد. این بیماری، سومین سرطان شایع در جهان است. درک تجارب معنوی این بیماران می‌تواند نقش مهمی در بهبود مراقبت‌های چندبعدی و انسانی ایفا کند.
هدف: این پژوهش، با هدف واکاوی تجربۀ زیستۀ معنوی در مردان مبتلا به سرطان کولون انجام شد.
روش: پژوهش حاضر از نوع کیفی و با رویکرد پدیدارشناسی تفسیری طراحی شد. مشارکت‌کنندگان پژوهش، شامل تمامی مردان مبتلا به سرطان کولون مراجعه‌کننده به بیمارستان امید مشهد در سه‌ماهه اول سال 1404 (حدود 60 نفر) بود. نمونه‌گیری هدفمند با روش گلوله‌برفی تا اشباع داده‌ها ادامه یافت، پس از مصاحبه با ۹ شرکت‌کننده، اشباع حاصل شد؛ داده‌ها از طریق مصاحبه‌های عمیق نیمه‌ساختاریافته گردآوری شدند. هر مصاحبه بین ۴۵ تا ۹۰ دقیقه طول کشید و حول محور تجربۀ زیستۀ معنوی از زمان تشخیص بیماری تمرکز داشت. تحلیل داده‌ها با روش تحلیل پدیدارشناسی تفسیری (IPA) انجام گرفت. برای اعتباربخشی یافته‌ها از یادداشت‌برداری بازتابی پژوهشگر، کدگذاری مستقل توسط دو تحلیل‌گر و محاسبه توافق میان ارزیابان و بازبینی مشارکتی با شرکت‌کنندگان استفاده شد.
یافته‌ها: تحلیل تجارب زیستۀ مشارکت‌کنندگان، منجر به شناسایی «جوهر تجربۀ معنوی» در قالب چهار مضمون محوری گردید: معنویت فردی و الهی (تجربۀ حضور خداوند به‌عنوان منبع بی‌پایان قدرت)، معنویت فردی و خودمحور (مواجهه با معنای جدید زندگی در سایه بیماری)، معنویت اجتماعی و حمایت خانوادگی (تجربۀ معنویت در بستر روابط انسانی) و معنویت وجودی و طبیعت‌گرایی (احساس یگانگی با جهان هستی).
نتیجه‌گیری: یافته‌های این پژوهش نشان داد، تجربۀ معنوی در مردان مبتلا به سرطان کولون، پدیده‌ای ایستا نیست، بلکه فرآیندی پویا از «معنابخشی مجدد به زندگی» در مواجهه با بحران است. این بیماران با تعریف مجدد رابطۀ خود با منابع قدرت (خدا، خود، دیگران و طبیعت)، رنج جسمانی را به فرصتی برای رشد وجودی تبدیل می‌کنند. سیاست‌گذاران حوزه سلامت می‌توانند با بهره‌گیری از این مضامین به‌جای تمرکزِ صرف بر درمانِ جسمی، به بازسازی دنیای معنایی بیمار کمک نمایند.

چکیده English

Background: Colon cancer, as a complex disease, severely affects the physical, psychological, social, and spiritual dimensions of patients. It is the third most common cancer globally. Understanding the spiritual experiences of these patients can play a vital role in improving multi-dimensional and human-centered care.
Aim: The present study aimed to explore the spiritual lived experience of men diagnosed with colon cancer.
Method: The present study was a qualitative research designed with an interpretative phenomenological analysis (IPA) approach. Research participants consisted of all men diagnosed with colon cancer who referred to Omid Hospital in Mashhad during the March, April, and May 2025 (approximately 60 individuals). Purposive sampling was conducted using the snowball technique until Data saturation was achieved; saturation was reached after interviews with 9 participants. Data were collected through in-depth semi-structured interviews. Each interview lasted between 45 and 90 minutes and focused on the lived spiritual experience since the diagnosis of the disease. Data analysis was performed using the interpretative phenomenological analysis (IPA) method. To enhance the credibility of the findings, strategies including researcher’s reflexive journaling, independent coding by two analysts with calculation of inter-coder agreement, and member checking with participants were employed.
Results: Analysis of the participants' lived experiences led to the identification of the "essence of spiritual experience" within four core themes: Individual and Divine Spirituality (experiencing God's presence as an infinite source of power), Individual and Self-Centered Spirituality (encountering a new meaning of life in the shadow of illness), Social Spirituality and Family Support (experiencing spirituality within the context of human relationships), and Existential and Naturalistic Spirituality (a sense of oneness with the universe).
Conclusions: The findings demonstrated that spiritual experience in men with colon cancer is not a static phenomenon but a dynamic process of "re-meaning life" in the face of crisis. By redefining their relationship with the sources of power, these patients transform physical suffering into an opportunity for existential growth. Health policymakers can utilize these helping to reconstruct the patient's semantic world rather than focusing solely on physical treatment.
